If you are missing a prerequisite for a course you would like to register for or you would like to take a prerequisite concurrently with the desired course (co-requisite), you must complete and submit this form. Please be advised that a waiver of prerequisite does not act as a course substitution. Once you have submitted this form, we will pass it on to your advisor and the class instructor for their permission. If everyone approves, we will register you in the class. You will receive an email confirmation from us either way.
