Calgary Hailstorm Impact Study
About the study:
- This study will help the community to document the impacts of the June 13, 2020 hailstorm in Northeast Calgary.
- The findings can be used to inform future disaster preparation and recovery policies/practices by insurance, government, and community organizations.
Informed Consent:
This consent form is only part of the process of informed consent. If you would like more details about something mentioned here, or information not included here, you are welcome to ask. Please take the time to read this document carefully and to be sure you understand any and all accompanying information.
The Ambrose University Research Ethics Board has approved this research study. This project is funded by The Co-operators.
What Will I Be Asked to Do?
- You will be asked to answer questions about your experiences during and after the June 13, 2020 hailstorm, and how all of this made you feel.
- This survey will take about five minutes to complete.
- This survey is completely voluntary (you can say no if you do not want to do it) and all questions are optional.
- You can withdraw from the study at any time without penalty.
- Doing this survey, or not doing this survey, will not result in changes to your insurance, benefits, or support that you receive from an organization.
What Type of Personal Information Will Be Collected?
- No personal identifying information will be collected in this study.
- We will ask non-identifying demographic information about you.
- We will ask about your experiences of the hailstorm.
Are there Risks or Benefits if I Participate?
- You will not encounter any risk more than your daily routine.
- You might have to think about your feelings on that day or after, and this might cause emotional stress and discomfort.
- A list of low-cost/free mental health resources will be provided.
- There will be no direct benefits to you. An indirect benefit will be the learning from the study that impacts policies/practices in the event of something like this happening again.
- The Co-operators insurance company is funding this project. Participation in this project will result in no promised action from The Co-operators.
What Happens to the Information I Provide?
- Participation is completely anonymous (we won’t use information to identify you personally).
- Participation is completely confidential (no one except the researcher, community researcher and their supervisor will be allowed to see or hear any of the answers to the survey)
- You are free to stop participation at any time during the study, and all data that you provided will be destroyed.
- The survey will be protected in a password protected folder.
- All data will be destroyed after 5 years.
Answering "yes" to the question (below) indicates the following:
- You understand to your satisfaction the information provided to you about your participation in this research project
- You agree to participate in this research project of your own free will
In no way does this waive your legal rights nor release the investigators, sponsors, or involved institutions from their legal and professional responsibilities. You are free to withdraw from this research project at any time. You should feel free to ask for clarification or new information throughout your participation.
