Exam Accommodation Request (EAR) form here.

  • Exam Accommodations can only be requested by students who have an active registration with the Accessibility & Student Support Office.
  • A form must be submitted for each exam that a student wants to use their previously approved accommodations on.
 

A Quick Guideline

An Exam Accommodation Request Form (EAR) must be submitted for both Online and In-Person exams.

The EAR form is due a at least 7 days (not including holidays) before the date the class is writing the exam or we will not be able to accommodate your request.  Final exam accommodation requests may need to be submitted earlier than 7 days before your final exam - please make note of the close date for the current semester at the top of the EAR form.

It is encouraged to submit an EAR form as soon as you know the date and time of an exam.

After submitting an EAR, students will receive an email, approximately 24 hours in advance of the exam, with the following information:

  • In-Person exams: Time and location where you will be writing the exam and accommodations you have been approved to utilize.
  • Online exams: Confirmation of access to accommodations and accommodations you have been approved to utilize.
 

Rescheduling an Exam

Written permission (email is appropriate) is required from your professor if you need to take an exam at a different time than the rest of the class.

Appropriate reasons for making a request:

  • Extra time needed conflicts with another exam.
  • Previous approval for no two exams on the same day.
  • Medically documented reason exam cannot be taken at scheduled time (including advice from AHS in regard to Covid-19).
 

24-hour rule

An exam will be rescheduled if no exam is received from your professor 24 hours before you are scheduled to take the exam. We recommend that you remind your professors to submit exams to Accessibility Services for processing.

If rescheduling is required, a new date will be set by the professor in coordination with Accessibility Services’ availability to accommodate.

With less than 24 hours’ notice, an exam cannot be moved to a different date and time under any circumstance.

This 24-hour rule is in place to ensure that Accessibility Services has enough time to book a room, hire a proctor, convert the exam into the student’s accommodated format, and prepare the student’s accommodated technology.
